No one has reported these issues before, but some investigation indicates there may be several things in play here simultaneously:

The type menu in "alternate names" was broken due to an introduced coding errors (ie. it worked until we accidentally broke it). It is actually saving properly but on record load the drop-down is not set. I just fixed this is development and will push to trunk over the weekend. I apologize for this problem. It was basically a typo.
I am not seeing any issues with the media representation upload or its status drop-down. They all work as expected.
I am not seeing any problems with internal notes
The representation "is primary" drop-down is functioning as expected but playing with it made me realize that the current functionality can be confusing and seem broken in certain cases. The short story is: only one representation attached to an object can be set to "primary" at a time. If you set two to "yes" one will be set to "no" automatically and this can seem like it's ignoring you. We're going to discuss some changes to make this work in a more obvious and intuitive way, but it is functioning.

We are not having problems with dimensions fields, etc., and there was a systemic saving issue we'd have heard since many people use those fields. (By contrast, few projects use those alternate name "type" drop-downs so it doesn't surprise me they could be broken and no one complained)

So I am thinking this may be a combination of some real bugs and some kind of strange problem with your set up.

One possible issue that has manifested itself in the past with symptoms similar to what you describe stems from the use of Suhosin (a PHP security package), and possibly mod_security (an Apache module). Are you running either on your server?

Is there any way for me to get access to your system to take a look?
